The three levels of emergency preparedness prevention are each an important part of being ready for a disaster. The planning involves the time before the disaster, the acute disaster scene, and the long term management of the disaster survivors (Rittenmeyer, 2007). At the disaster scene, a triage color code system is used to organize and prioritize patients and the level of care they require. Tertiary Prevention Tertiary prevention, or disaster recovery, begins when the initial crisis is over and involves long term support for the needs of the population affected by the disaster. Activities that take place during the recovery phase include rebuilding affected infrastructure, hospitalization for the injured, rehabilitation and therapy to cope with the disaster. These will vary according to the type of disaster that has occurred. Rittenmeyer (2007) states that during the recovery phase the effectiveness of the disaster plan should be evaluated and the plan then altered based on the findings. Triage Color Code System In the event of a disaster, the triage color code system is beneficial for three major reasons (Klein & Nagel, 2007). First, triage determines who needs rapid medical care. Next, triage reduces the amount of patients sent  to hospitals by separating minor versus major injuries. Thirdly, triage distributes casualties among available medical facilities to keep any one facility from being deluged with patients. The system most widely used during a disaster triage is the IDME color code system. The mneumonic IDME stands for the levels of acuity of the patients. Each level is assigned a color. They are as follows according to Husted (2012): I–Immediate (Red); D–Delayed (Yellow); M–Minimal (Green); and E–Expectant (Black). The categories have criteria that the responder should be familiar with in order to triage the patients into the appropriate color. Based on the patient’s level of injury, each is given a triage tag, commonly placed on the wrist. Using this system, patients are treated in order of the urgency of their injury. The red category is reserved for critical patients. These patients are seriously injured but do have a chance of surviving. The yellow category is for patients that need first aid but should not deteriorate rapidly if care is not immediate. The green category is for patients that are considered the â€Å"walking wounded†. These patients may have minor injuries such as abrasions or contusions and can either self treat or be taken care of by a someone without medical training. The final category is the black which is for patient who is unresponsive and without a pulse or has a catastrophic chest or head injury (Husted, 2012). Types of Disasters Three types of disasters that could occur are exposure to biologic, chemical and radioactive agents. Synthesis of Isatin Based Caspase Inhibitors DESIGN AND SYNTHESIS OF ISATIN BASED CASPASE INHIBITORS FOR RUTHENIUM CAGING APPLICATIONS KASUN CHINTHAKA RATNAYAKE ABSTRACT Apoptosis is the energy dependent programmed cell death. Improper function of apoptosis could lead to diseases such as cancers, strokes, alziemer’s disease. Caspases are the enzymes involved in the later stage of this process. Peptidyl and non-peptidyl caspase inhibitors have been synthesized recently. One of these non-peptidyl compound classes which consist of pyrrolidinyl-5-sulfo isatins have showed a greater potency against executioner caspases, caspase-3 and -7. According to literature and for further caging studies, two compounds were designed, synthesized and evaluated their inhibition against caspase-3 in this study. The analog in which its N-1 position alkylated with a 4-methyl pyridine moiety (7) showed a higher inhibition than the analog in which its N-1 alkylated with cyanoethyl group (8). Thus, the compound  7  was selected for further caging studies with ruthenium. Chapter 1: Introduction 1.1 Apoptosis and Caspases Apoptosis is the process of programmed cell death. This is a significant cellular process which is directly co-related with embryogenesis, immune system, ageing and various diseases including cancers, stroke, myocardial infarction and neurodegenerative disorders.1 Caspases (cysteinyl dependent aspartate directed specific proteases) are the enzymes involved in the later stage of apoptosis. Caspases are divided to different classes according to their role played in the signaling cascade of apoptosis. Caspases 6, 8, 9 and 10 are involved as initiators and caspases 2, 3 and 7 are identified as executioner caspases in the signaling cascade.2The caspases 1, 4 and 5 are found to be non-active in the cell death process. 1.2 Caspase inhibition and modified isatin sulfonamides as caspase inhibitors Caspases play a significant role in both inflammation and apoptosis. Extensive researches have been conducted on caspases and their functions because they act as potential targets in drug discovery. Various inhibitors of Caspase have been made. These inhibitors could be categorized as non-peptidyl and peptidyl based compounds. A greater selectivity could be achieved when non-peptidyl inhibitors are used for different types of caspases. Isatin sulfonamides have showed inhibition on executioner caspases (caspase-3 and -7) in recent studies. In 2000, Lee and researchers reported the x-ray structure of caspase-3 with an isatin analog, 1-methyl-5-(2-phenoxymethyl-pyrrolidine-1-sulfonyl)-1h-indole-2,3-dione (a) bound to the active site of the enzyme (Figure 1).3 Modifying isatin sulfonamide analogues with pyrrolidine groups have shown significant effect on caspase inhibition.4 For example, various pyrrolidinyl-5-sulfo isatins have been shown inhibition to caspases, 3 and 7 (Figure 2). These isatin sulfonamide analogs are modified using structure activity relationships and performed these biological assays. The following isatin sulfonamides have shown to be inhibit caspase-3. The stereochemistry of substituted pyrrolidine moiety, cyclic vs acyclic ring structures and ring sizes have been examined for these inhibition studies (figure 3).5 1.3 Ruthenium complexes for caging applications Ruthenium compounds have been reported as significant candidates for caging applications. Light activation of these metal complexes has been extensively studied. Recently, neuroactive biomolecules as well as small molecular enzyme inhibitors have been reported to be caged with these ruthenium complexes. Spatial and temporal release of these caged molecules upon light activation gives insight to develop new tools that could be used to treat various diseases in biological systems. In this study Ruthenium polypyridyl compounds are used in future studies since they have been considered as excellent candidates for caging application of small molecules. Chapter 2: Results and Data 2.1 General considerations All reagents were purchased from commercial suppliers and used as received. Several factors were considered in the designing process of these analogs. First, higher caspase inhibition was considered. Use of specific stereochemistry in the pyrrolidine moiety is important since S-alkoxypyrrolidine is more potent than its R-stereoisomer which shows almost no potency against caspase-3. It is reported that methoxymethyl pyrrolidinyl analogs show higher cell toxicity than phenoxymethyl pyrrolidines, thus methoxymethyl pyrrolidine analogs were chosen for further studies. When considering the Ruthenium caging studies, the chosen analogs should contain a group which has a higher binding affinitiy towards Ruthenium. Therefore, pyridyl and cyano groups were selected to incorporate in these isatin sulfonamide analogs. These groups are chosen to be attached to N-1 position of isatin sulfonamide analog. It has been reported that higher alkyl chain on N-1 position could increase th e inhibition. Therefore 4-methylpyridine and cyanoethyl groups were selected to attach on N-1 position of these analogs and compounds 7 and 8 are designed (Figure 3). 2.3 Synthesis of designed isatin sulfonamide analogs The designed analogs were synthesized using literature and modified procedures5, 6, 7 (Scheme 1). The compound 5 was synthesized as the precursor for the final analogs 7 and 8. The compounds 7 and 8 were synthesized using modified and optimized procedures (Scheme 2 and Scheme 3). 2.4 Enzyme Inhibition Assay Caspase-3 inhibition assay was performed for compounds 6 and 7 according to the literature procedure.2 Compound 6 was found to be more potent (IC50 = .. ) of than compound 7 (IC50 = ..). Thus, compound 6 was selected for further caging studies with Ruthenium bipyridine complexes. 2.5 Experimental 2.5.1 Sodium 2,3-dioxoindoline-5-sulfonate (1) Isatin (10 g, 0.068 mol) was added carefully to a stirred solution of 20% SO3/H2SO4 (20 mL) at -15 °C. The reaction mixture was gently warmed up to 70  °C with stirring. Reaction mixture was stirred at 70  °C for another 15-20 min. The reaction mixture was carefully poured on to crushed ice and let ice to melt and then 20% NaOH was added to the reaction mixture (pH=7). The flask containing reaction mixture was kept in an ice bath to induce precipitation of the desired product. The solid was filtered, washed with ice-cold water and dried to give red-orange crystalline solid. The 1H-NMR data was compared and matched with literature data. Yield: 14.48 g (0.051 mol. 75%) 2.5.2 2,3-dioxoindoline-5-sulfonyl chloride (2) Sodium 2,3-dioxoindoline-5-sulfonate dihydrate (2 g, 70 mmol) was dissolved in tetramethylene sulfone (10 mL) under Argon environment at 60-70  °C and phosphorus oxychloride (3.36 mL, ) was added dropwise. The reaction mixture was stirred for 3 h. The reaction was cooled to room temperature and kept in an ice bath. Then ice-cold water was added to the reaction mixture carefully. A precipitate was formed, filtered, washed with ice-cold water and dried used without further purification. The desired compound is yielded as a bright yellow solid. The 1H-NMR data was compared and matched with literature data. Yield: 1.58 g (64 mmol, 92%). 2.5.3 Tert-butyl (S)-2-(methoxymethyl)pyrrolidine-1-carboxylate (3) To a solution of N-Boc-L-prolinol (5.0 g, 25 mmol) in THF (25 mL) at -78  °C, Sodium hydride (60% in mineral oil) (960 mg, 40.0 mmol) was added and stirred for 10 min. Then methyl iodide (2.65 mL, 42.5 mmol) was added dropwise and reaction was stirred for 4h at -78  °C and additional 16 h at RT. Then NH4Cl was added until all H2 evolved and EtOAc was added. The organic layer was washed with water and sat. NaCl, dried over anhyd. Na2SO4 and concentrated to give a pale yellow oil and purified with petroleum ether: ether (9:1) to give a colorless oil. The 1H-NMR data was compared and matched with literature data. Yield: 4.986 g (23.16 mmol, 92%) 2.5.4 (S)-2-(methoxymethyl)pyrrolidine (4) To a solution of tert-butyl (S)-2-(methoxymethyl)pyrrolidine-1-carboxylate (4.98 g, 23.07 mmol) in DCM (40 mL), TFA (25 mL) was added dropwise over 30 min at 0  °C. The reaction was warmed to RT and stirred for additional 1.5 h. The reaction mixture was added to 150 mL of 10% NaOH solution and extracted with DCM (50 mL x 3), dried over anhyd. Na2SO4 and concentrated to obtain a pale yellow oil. The 1H-NMR data was compared and matched with literature data. Yield: 2.657 g (23.07 mmol, 100%) 2.5.5 (S)-5-((2-(methoxymethyl)pyrrolidin-1-yl)sulfonyl)indoline-2,3-dione (5) The compound (1) was synthesized according to procedure reported by Harvan et al.1 To a stirred solution of 2,3-dioxoindoline-5-sulfonyl chloride (2 g, 8.153 mmol) in 1:1 THF/CHCl3 (80 mL), a solution of (S)-2-(methoxymethyl)pyrrolidine (1.033 g, 8.968 mmol) and DIPEA (2.84 mL, 16.310 mmol) in CHCl3 was added dropwise under Argon environment and stirred for 1 h at 0  °C. The reaction stirred for additional 1 h at RT. The reaction mixture was concentrated and purified using 1:1 EtOAc:Petroleum ether and isolated as bright yellow crystals. The 1H-NMR data was compared and matched with literature data. Yield: 1.185 g (36.53 mmol, 45%) 2.5.6 4-(bromomethyl)pyridine hydrobromide salt (6) Pyridin-4-ylmethanol (5.0 g) was dissolved in 48% HBr (50 mL) and refluxed for 24 h. The caged ruthenium complexes could be subjected for light activation experiments where IC50 of this complex under light and dark conditions could be determined and the dark to light inhibition ratio could be explored. Then cell toxicity studies could be done in order to explore the ability of these ruthenium complexes for prevention of apoptosis in biological systems. These combined experiments and results could lead to the final goal of this research study which is the development of novel tools to prevent apoptosis in biological systems.

I turned off the car and took a deep breath. Looking slowly up into the pink sky, I began to watch the golden sun go to sleep. The beach seemed deserted, quiet, but peaceful. I opened my door and put my feet out on the soft sand. I started taking my shoes off, then my socks. I threw them in the passenger seat, and then shut the door. I looked out over horizon of the lake and started walking towards the still water. With each step I took, I could feel the warm sand crunch between my toes. Then suddenly, a sharp rock, but not sharp enough to break the skin. Closer and closer to the calm water, I began sinking deeper in the sand. It was comforting, the silence, tranquility, and warmth of the faint sun. There is a slight breeze, warm, but cold and lonely. I could smell the scent of fish blowing through my hair and body. The sun was still fading, slowly but surely the day was almost over. About half of it is gone now. I could see shades of blue, red, purple, and pinkish-yellow. They were mixed with puffy clouds that lined the beginning of the sky and the end of the water. I noticed the darker shades on the bottom of the lower clouds. I stopped walking and looked up at the faint stars. The seagulls were flying overhead. They were screeching and swooping at the water. I started to wish I were one of them, flying free without any restrictions or limits. I listened to their voice, the screech. Deep down in I could understand what they were saying. I can't explain it, but I was so in love with the moment I thought I saw things as they did. I was in company of animals that had no concept of time, and no worries, and I was contempt with that. (chapter 1) ‘ a few miles south of Soledad, the Salinas River drops in close to hillside bank.’ whereas in this chapter, a description of the bunk house is given, it lacks comfort. ‘ Inside the walls were whitewash ed and the floor unpainted.’ This is a starch contrast with the previous scene. The detailed description of the setting is always the place in which the action happens. In the novel tension is created through a series of ominous signs. I have noticed a particular order in which it is created in . Anger , then a calmer atmosphere which is then broken by anger , rage or confusion. Tension, anxiety and rage is created when Curley ( the boss’s son) enters the room. ‘he glanced coldly’ this suggests that he is unfriendly and a threat. ‘ he stiffened his fists†¦his glance was at once calculating and pugnacious.’ when Curley stiffened his firsts, this showed that he was looking for a fight , the readers already know that he is an aggressive character and confronts people with this. ‘ He hates big guys. He’s alla time picking scraps with big guys. Kind of like he’s mad at’em because he aint a big guy.’ The readers suddenly realise that there would be a future problem with Lennie and Curley, further on in the novel. Curley seems set on proving that he is a big man, in all but size. His success in the past as a boxer is an obsession, so every person Curley meets is a possible opponent. He sees life and everyone he faces as a competition. Curley’s authority on the ranch triggers tension as most of the ranch hands are frightened of him and do not want to confront him as there would be trouble. During chapter two, my sympathy fell towards the stable buck, Crook. He is a victim of racial prejudice. the way Candy gossiped about Crook to George and Lennie was unjust ,they would normally refer to Crook as ‘the nigger’. Nowadays that is a racial offence. They way Candy said that Crook has no respect he was the lowest of the low. ‘Nice fella too. Got a crooked back where a horse kicked him. The boss gives him hell when he’s mad. But the stable buck don’t give a damn about that.’ Despite Crook being discriminated against, he is a proud, independent and intelligent as he reads a lot. But none of these admirable features stop the boss from giving him ‘hell’. As he is not a respected character and he has no authority.

Substances fit Into three different categories, with more than one category fitting some substances. The three categories are: Stimulants, Depressants and Hallucinogens. I will proceed to list the main substances, their effects, how they are taken and the category they fit into: Alkyl Nitrates (poppers, amyl nitrate, butyl nitrate, gibbously nitrate): Category:Hallucinogens Effect: Brief but intense head rush, flushed face and neck, effects fade after 2-5- minutes Form and how taken: Vapor which is breathed in through the mouth or nose or from a small bottle or tube. Amphetamines (speed, waltz, poet, belly, sulfate, crystal meet): Category: Stimulants Effect : Excitement, the mind races and users feel confident and energetic, suppresses app etite, smoking crystal meet will produce a more intense rush Form and how taken: Grey or white powder that is snorted, swallowed, smoked, injected or dissolved in a drink.Base is a strong version of the powder form and is white or yellowish in color and is usually swallowed or injected. Tablets which are swallowed. Crystal meet is crystalline and is smoked or injected. Anabolic Steroids (rods): Category: Users say the drugs make them feel more aggressive and able to train harder, helps build up muscle with exercise, helps users recover from strenuous exercise Form and how taken: Tablets that are swallowed or liquids that are injected BGP – Phenylalanine (BIZ, pep, pep love, pep twisted, pep stones, the good stuff, exodus, frenzy): Category:Causes effects similar to amphetamine but not as strong. When combined with other types of prissiness the effects seem to be compounded which gives a greater sense of euphoria – (effects can differ from pill to pill due to the nature of the chemical make-up) Form and how taken: Pills – many shapes, forms and colors – Often have Imprints such as fly, crown, heart and can often be sold as ecstasy Cannabis (Marijuana, weed, puff, skunk, blow, phonic, draw. Soapbox): Category: Users feel relaxed and talkative. May cause hilarity. Usually brings on cravings for food.Strong skunk can cause hallucinations. Form and how taken: A solid dark lump known as resin/hash or leaves, buds, stalks and seeds, or a sticky dark oil. Can be rolled either with or without tobacco in a Joint or smoked in a pipe, or eaten in a cake or biscuits. Cocaine and Crack (Cocaine, Charlie, coke, crack, wash, rock, white): Category: Sense of well-being alertness and confidence. For cocaine the effect can last for around 30 minutes, users are often left wishing for more. Crack has the same effects as cocaine but is much more intense and a much shorter high with a ore intense craving for more.Form and how taken: Cocaine – white powder that is snorted up the nose, dabbed on gums, swallowed and sometimes dissolved and injected. Crack – small raisin sized crystals which are smoked in a pipe Heroin (smack, gear, brown, H): Depressants Strong feeling of warmth, contentment and well-being (happy bubble) Form and how taken: Usually brown, sometimes white powder. Ecstasy (E, doves, pills, burgers, disco biscuit, maturities, MADAM): Category: Users feel alert and in tune with their surroundings.Sound, color and emotions seem more intense. Users may dance for extended periods. Effects may last for 3 to 6 hours. Form and how used: Tablets of different shapes, size and color – often with some kind of logo on. Pure MADAM (the main ingredient in ecstasy pills) is a brown colored crystal and is crushed and snorted. Gases, glues and aerosols (lighter gas, aerosols containing products such as hairspray, deodorants and air fresheners, tins or tubes of glue, some paints, thinners and correcting fluids): Category:Hallucinogens Similar to being very drunk, also thick headed, dizzy, giggly, dreamy, and hallucinations Form and how used: Sniffed or breathed into the lungs from a cloth or sleeve. Gas products are sometimes squirted directly into the back of the throat. Alcohol: Disinherited, euphoria, loss of coordination and unconsciousness Form and how used:Drunk as a liquid Astatine (K, special K, vitamin K): in severe cases Form and how used: Legally produced as a liquid, illegally produced as a pill or grainy white powder – usually snorted or prepared for injection ND injected.Painlessness's (Valid, digamma, tempera, neutralize, planetarium, yellows, blues, smarmiest, bonzes, reopen, trains, Jellies, valise, mommies, roofers, downers): Long acting bonzes such as valid/digamma are commonly used for stress and anxiety management. Short acting ones such as tempera are often used for help with sleep. Used in detoxification w here there is risk of fitting. All cause a calm relaxed feeling or bring on sleep. Form and how used: Either swallowed as a pill, injected or used in a suppositoryOpiates (Methadone, codeine, Dehydrogenate (DIF 18), Diction, Petitioned, Opium, Morphine, Playful, Subtext – partial opiate): Category: Can be divided into long acting and short acting. SHort acting can depending on dose, give feelings of contentment and relief of distress that is also offered by heroin. Longer acting opiates such as methadone cause less euphoria and are mainly used to relieve symptoms of withdrawal. Form and how used: Pills – swallowed, linctuses – swallowed, enunciable preparations. GHB (GHB, Liquid Ecstasy, gammahydroxybutyrate, RE, rib, blue Juice, roofless, liquid E):Small amounts may cause happiness and distribution. Increasing amounts brings out depressant effects. Sometimes used for bodybuilding. Induces sleep and is often associated with rape. Form and how used: Odorless slig htly salty liquid. Concentration is variable so may be difficult to measure dosage – often mixed with a drink LSI (Acid, trip, tab, blotter, stars): Time distortion, perceptual changes, visual colors and patterns Form and how used: Usually a small tab of paper with a colorful pattern on – usually eaten or wiped around gums
